During a focused urinary assessment ,the nurse should ask to the patient about painful urination and also about any changes in the characteristics of urination that is it diminished , excessive because these information help in understand the presence or absence of urinary disorders.

There are two type of data that can be collected - subjective data which included reports from the patient and objective data which includes data from physical examinations and medical tests.

Data that collected such as color of their urine, about history of urinary tract infection, burning , frequency presence of blood in urine, sediment , odor with urine and genital health issues. Nurse also ask for nocturia and incomplete bladder emptying.
