First of all we need to find what y equals to have it in slope intercept form.
y-x=5
y=x+5
As you can see, our slope has to be one, and our y-intercept is 5.

Isolate the y. Add x to both sides
Y - X (+X) = 5 (+X)
Y = 5 (+X)
Note however, that 5 is the b (constant), and so it needs to behind 1(x) (the slope and variable.)
